No Basis for Charges After Man’s Nose Broken in Bradford Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a South Simcoe Police Service officer committed a criminal offence in connection with the fractured nose suffered by a 55-year-old man during his arrest at a Canada Post outlet in Bradford. On December 20, 2025, staff contacted police to have a belligerent man removed from the premises.

Toronto Police Officer Charged with Manslaughter in Connection to Niagara Falls Police-Involved Shooting

The Director of the Special Investigations Unit, Joseph Martino, has reasonable grounds to believe that a Toronto Police Service officer committed a criminal offence in connection with the shooting death of a 40-year-old man on July 30, 2025. Director Martino has caused a charge to be laid against the officer.

SIU Determines No Grounds to Charge Officer in Relation to Fatal Mississauga Shooting

A 19-year-old male died on December 11, 2025, the result of gunshot wounds inflicted by a Peel Regional Police officer. The SIU was notified of the incident and initiated an investigation. The investigation is now concluded.  On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s death.

SIU Concludes Investigation into Arrest of Woman in Hamilton

A 58-year-old woman was diagnosed with a serious injury following her arrest by Hamilton Police Service officers on September 7, 2025. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with woman’s arrest and injury.

SIU Concludes Investigation into Man’s Serious Injuries Following Struggle on London Apartment Balcony

In the morning of December 11, 2025, London Police Service officers attended an apartment to compel a man to attend hospital for a psychiatric examination. An individual who knew the man had become increasingly concerned for the his well-being. The man refused to leave his apartment and threatened police that he would jump from the balcony and harm them or himself if they entered his residence. Negotiations continued. Emergency Response Unit members proceeded to rappel from the roof to the man’s balcony, to prevent him from using it to jump from the building. As the officers were reaching the balcony railing in question, they were confronted by the man. With an aluminum baseball bat in hand, the man began to swing at the officers as they were still hanging from their rappel lines, striking one of them in the left hand. A struggle ensued on the balcony, and the man was eventually arrested. He was taken to hospital where he was diagnosed with multiple facial fractures. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s injuries.

SIU Concludes Investigation into Man Fatally Shot at Toronto Go Train Station

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Toronto Police Service officer committed a criminal offence in connection with the shooting death of a 32-year-old man at the Danforth GO Station. On December 6, 2025, the man was fatally shot by an officer while he drew his BB gun and swung it in the officer’s direction.

SIU Concludes Investigation into Fatal Police-Involved Shooting of Man with Axe in Sudbury

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Greater Sudbury Police Service officer committed a criminal offence in connection with the fatal shooting of a 33-year-old man. On December 5, 2025, officers were dispatched to the area of Clinton Avenue and Queen Street after a man with a knife attempted to break into a home.

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Hamilton Police Service officer committed a criminal offence in connection with the fractured nose and right orbital bone suffered by a 27-year-old man during his arrest. On December 4, 2025, officers were called to a residence near Wentworth Street and Main Street East in relation to a domestic abuse investigation. The man also had warrants for his arrest on firearm-related charges.

SIU Concludes Investigation into Man’s Foot Fractures in Hamilton Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe Hamilton Police Service officers committed a criminal offence in connection with a fractured right foot suffered by a 24-year-old man during his arrest. On December 3, 2025, officers were called to a trailer parked in the rear of a property at Wellington Street North and Burlington Street East for the assault of a woman. Officers forced their way into the residence after hearing a female scream. Officers grabbed the man and pulled him forward out of the trailer, when the man landed awkwardly on his right foot. The man was handcuffed while in the prone position on the ground.

SIU Concludes Investigation into Man’s Drug Overdose in Pembroke OPP Custody

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe an Ontario Provincial Police officer committed a criminal offence in connection with the drug overdose suffered by a 44-year-old man while in custody. On December 2, 2025, officers were called to a residence in Pembroke for a disturbance involving the man.

SIU Concludes Investigation into Man’s Injury Suffered During RIDE Check in Brampton

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Peel Regional Police officer committed a criminal offence in connection with the possible concussion suffered by a 39-year-old man during his arrest in Brampton. On November 28, 2026, the man was driving home from work on Queen Street East and Highway 410 when he was pulled over by officers for a RIDE program and refused to do a breath test. The man walked to the driver’s door of his pickup truck, and the officer directed the man to the rear area for safety away from a live lane of traffic. The man fell and he tried to stand up as officers struggled to handcuff him. Body worn camera footage showed officers’ attempting to wrestle control of the man’s arms. Director Martino was unable to reasonably conclude the force used was more than was necessary to arrest the man.

SIU Concludes Investigation into Highway 400 Collision

In the morning of November 28, 2025, a man was operating his vehicle on Highway 400 when it began to lose power. The 29-year-old driver managed to maneuver the vehicle to the second-lane from the right before it stopped on the road. He was several hundred metres north of Highway 401. Activating his emergency flashers, the man stepped out of the vehicle and popped the front hood to investigate what had happened. The man had just closed the hood when an OPP cruiser slammed into the rear of the vehicle. The impact propelled the man backward on the highway. He suffered fractures of the right leg and right hand. A 25-year-old male passenger in the vehicle also suffered multiple fractures. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the collision.

SIU Concludes Investigation into Man’s Death Following Self-Inflicted Stabbing in Waterloo

In the evening of November 27, 2025, Waterloo Regional Police Service officers were dispatched to a home in Waterloo after residents reported to police that a guest had retrieved a knife from the kitchen. Upon arriving at the residence, officers learned that the guest, a 19-year-old man, had stabbed himself in the abdomen. The officers located the man lying prone on the floor. Following his arrest, the officers sat the man on the floor and checked him for injuries. Paramedics arrived on scene and tended to the man. Within a short time, the man went vital signs absent. He was provided emergency care and transported to hospital where he was pronounced deceased. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s death.

SIU Concludes Investigation into Woman’s Arrest at Oastler Lake Provincial Park in Parry Sound

In the early morning of October 5, 2025, Ontario Provincial Police officers attended a campsite in Oastler Lake Provincial Park after a park warden witnessed a woman slap another individual in the face. Officers took custody of the woman and transported her to the Parry Sound Detachment. She was lodged in a cell and later released. The woman subsequently attended hospital and was diagnosed with two broken right-sided ribs. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the woman’s injuries.

SIU Concludes Investigation into Man’s Illness While Lodged in London Police Service Cell

In the afternoon of November 26, 2025, a 42-year-old man was arrested in connection with a stolen automobile. He was searched at the scene and transported to the London Police Service headquarters where he was lodged in a cell at about 5:45 p.m. He told police that he had consumed heroin a couple of hours before his arrest and explained he would become sick in his cell once the effects of the drug wore off. The man was monitored by special constables while in cells. He appeared to sleep for most of his time in custody. At about 7:40 a.m., the man had just woken when he started to vomit.  He was removed from the cell and transported to hospital where he was treated for opioid withdrawal. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s illness.

SIU Concludes Chatham-Kent Investigation

In the afternoon of November 26, 2025, a 34-year-old man was arrested for drug trafficking following a traffic stop by Chatham-Kent Police Service officers. A warrant authorizing a search of the man’s residence for evidence of drug trafficking was in effect at the time. An officer searched the man at the scene and confiscated a quantity of crack cocaine and fentanyl. The man was transported to the police station and subjected to another search of his clothing, this time with negative results. While lodged in a cell, the man began to vomit. He was transported to hospital where he was diagnosed with opioid withdrawal. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the events under investigation.

SIU Concludes Investigation into Constance Lake First Nation Collision

In the afternoon of November 25, 2025, two Nishnawbe Aski Police Service officers were on patrol in Constance Lake First Nation when they observed an individual operating a motorcross-type vehicle. Aware of a band council resolution prohibiting these types of motorcycles in the area, the officers activated their emergency lights to stop the motorcyclist. A short time later, the motorcycle rider lost control while attempting to negotiate a sharp left-hand curve, resulting in him falling onto his left side. The man picked himself up and briefly remounted the motorcycle before falling again. He was arrested. When he complained of pain to his left hand, he was transported to hospital where he was diagnosed with a fracture of a left finger. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Man’s Broken Elbow in Bracebridge Hospital Arrest

The Deputy Director of the Special Investigations Unit, Stacey O’Brien, found no reasonable grounds to believe an Ontario Provincial Police officer committed a criminal offence in connection with the left elbow suffered by a 31-year-old man in Bracebridge. On November 24, 2025, the man was discharged from the hospital and refused to leave so he was escorted out by security. He refused to leave the property. Police were called and arrested the man for trespassing.

SIU Concludes Investigation into Man’s Broken Nose After Thunder Bay Arrest

The Deputy Director of the Special Investigations Unit, Stacey O’Brien, found no reasonable grounds to believe a Thunder Bay Police Service officer committed a criminal offence after a 29-year-old man broke his nose. On October 9, 2025, an off-duty officer saw the man spray painting a business and attempted to arrest him for mischief. The man fled and fell to the ground. The officer secured him on the ground while he tried to get away.

SIU Concludes Investigation into Man Fatally Shot by Niagara Police Officer

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Niagara Regional Police Service officer committed a criminal offence in connection with the fatal shooting of a 38-year-old man. On October 22, 2025, the man died in an exchange of gunfire with police at a gas station in Niagara Falls. He was wanted for killing a woman the day before in Brampton, and abducting a child that resulted in an Amber Alert.

SIU Finds No Reasonable Grounds to Charge York Regional Police Officer Following Sexual Assault Complaint During Markham Arrest

On October 16, 2025, the Law Enforcement Complaints Agency contacted the SIU to report that they were in receipt of information in which it was alleged that an officer had sexually assaulted a 17-year-old male in the course of his arrest on September 1, 2025. The SIU initiated an investigation, naming a York Regional Police officer as the subject official. On Director Joseph Martino’s assessment of the evidence, he found no reasonable grounds to believe that the officer committed a criminal offence in connection with the complaint of sexual assault.

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe two Ontario Provincial Police (OPP) officers committed a criminal offence in connection with shooting their Anti-riot Weapon Enfields (ARWENs) at a 38-year-old man last November in the Bainsville area. He was not injured.

In the afternoon of November 12, 2025, a 71-year-old woman suffered a fractured right foot when the vehicle she was operating became involved in a collision with a Chevrolet Cruze on the bridge over the Hydro Canal. Prior to the collision, a Niagara Regional Police Service officer had been following the Chevrolet. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the woman’s injury.

No Basis to Proceed with Charges in Relation to Monkland Arrest

In the afternoon of November 13, 2025, Cornwall Police Service officers were on the lookout for the 51-year-old man who was wanted for a number of break and enters, and subject to an outstanding arrest warrant. Officers located him at a gas station in Monkland. Shortly after one of the officers approached the man and advised him he was under arrest, a struggle ensued.  With the assistance of three civilians on scene, the officers overcame the man’s resistance and handcuffed him behind the back. The man was transported to hospital where diagnostic imaging was unable to rule out undisplaced fractures to the left fourth and fifth ribs. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injuries.

SIU Concludes Investigation into Brantford Arrest of Man

In the evening of November 12, 2025, Brantford Police Service officers attended an apartment after a resident had contacted police to report that an inebriated man was in her apartment and she wanted him removed. The officers entered the apartment and approached the man in the living room. The man was seated on a sofa and speaking on the phone with a police call-taker. When the man walked towards the door, the officers took a hold of his arms. The man attempted to free his arms and the officers maintained their grip. Other officers intervened and the man fell forward to the floor, striking his head off the edge of a wall in the process. The man was handcuffed and taken into custody. Following his arrest, the man was transported to hospital and diagnosed with a reduced level of consciousness and a scalp laceration with emphysema and subgaleal hematoma. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s arrest and injuries.

SIU Concludes Investigation into Man’s Facial Fractures During Arrest by Halton Police Officers

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Halton Regional Police Service officer committed a criminal offence in connection with the facial fractures suffered by a 38-year-old man during his arrest in Mississauga. On November 4, 2025, the man, driving a pickup truck, attempted to flee from officers following a traffic stop for suspected impaired driving in Oakville. There was a 12-minute pursuit before it ended in Mississauga. During the arrest, the man was Tasered, pepper sprayed, punched, kicked and kneed.

SIU Concludes Investigation into Man’s Broken Orbital Bone in Toronto Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Toronto Police Service officer committed a criminal offence in connection with the fractured left orbital bone suffered by a 40-year-old man. It remains unclear whether the man was injured in an altercation with a police officer on November 2 or 3, 2025. The man was arrested on November 2, 2025, at house in the area of Ellesmere Road and Meadowvale Road in connection with a reported disturbance. During both his arrest and his time in custody the following day, the man was combative with police. Director Martino was satisfied that the police used no more force than was legally justified during the man’s arrest and period of custody.

SIU Concludes Investigation into Man’s Broken Rib in Toronto Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Toronto Police Service officer committed a criminal offence in connection with the fractured right rib of a 32-year-old man during his arrest. On November 2, 2025, officers were executing a search warrant outside a residence in the area of Kingston Road and Brimley Road in relation to firearm offences.  The man was taken to the ground and eventually handcuffed behind the back.

SIU Concludes Investigation into Napanee Arrest of Woman

On November 2, 2025, in Napanee, an OPP officer located a woman who was wanted for stealing a bottle of alcohol from a store the day prior. Told she was under arrest for shoplifting, the woman fled on foot, and the officer gave chase. A struggle ensued. With the assistance of another officer, the woman was eventually handcuffed. Following her arrest, the woman was transported to hospital where she was diagnosed with a fractured left rib. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the woman’s arrest and injury.

SIU Concludes Investigation into Fatal Essex County Collision Involving OPP Cruiser

On the morning of November 2, 2025, an OPP officer was travelling west on County Road 20 East en route to the OPP detachment in Essex. A distance east of Iler Road, a woman moved out onto the westbound lane from the northside ditch. The officer steered his cruiser to the right but was unable to avoid contact. The front of his vehicle struck the woman. The officer exited his cruiser and began to administer emergency first-aid. Additional officers arrived on scene, as did paramedics. The 19-year-old woman was transported to hospital where she was pronounced deceased. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that the officer committed a criminal offence in connection with the woman’s death.

SIU Concludes Investigation into Brampton Arrest

In the early afternoon of October 31, 2025, members of the Peel Regional Police Special Enforcement Bureau were in the parking lot of a Brampton restaurant waiting to arrest a man wanted for being in unlawful possession of a firearm. Upon leaving the restaurant, the man was confronted by officers and shortly after taken into custody. The man was subjected to knee strikes and a conducted energy weapon discharge on the ground prior to being handcuffed behind the back. The 36-year-old man was transported to hospital following his arrest and diagnosed with a fracture of the right clavicle. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Firearm Discharge by Toronto Officer in Mississauga Parking Garage

On October 29, 2025, a Toronto Police Service officer discharged a firearm in the course of a police operation in Mississauga involving a stolen vehicle. The driver of this vehicle remains at large and his identity is unknown to this date. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that the officer committed a criminal offence in connection with the shooting.

SIU Concludes Investigation into Oakville Arrest

Shortly before midnight, October 18, 2025, Halton Regional Police Service officers with the Tactical Rescue Unit were dispatched to an Oakville residence, with the intention to arrest a man under the Mental Health Act following a number of calls to police in which he was reported to have attacked motorists, brandishing a knife in one instance. The man refused to surrender to police.  Over the course of the next couple of hours, he would exit and re-enter his home through the front door, challenging the officers gathered by the road outside his house. At one point, officers were able to distract the man and apprehend him. The man was seen at hospital after his arrest and treated for a deep laceration to the scalp. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Toronto Arrest

On October 31, 2025, a Toronto store’s alarm had been tripped and the owner called police to report masked men burglarizing the business. The first officers to arrive on scene approached an SUV and ordered its occupants not to move. The SUV accelerated away from the scene. The officers approached the entrance of the business. They observed a masked man and ordered him to raise his hands. The man was arrested. The 30-year-old man was transported to hospital and diagnosed with a possibly fractured rib. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Man’s Arrest Inside Toronto’s Scotiabank Arena

On October 27, 2025, two Toronto Police Service officers were working a paid duty at the Scotiabank Arena when they noticed a  man being belligerent, and not leaving the arena when asked to do so by security. The officers walked over to intervene. One of the officers grabbed a hold of the man and started to escort him towards the exit. A scuffle ensued. The man was arrested and transported to hospital for treatment of a serious injury. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injury.

No Basis to Proceed with Criminal Charges in Sudbury Collision

In the early morning of October 26, 2025, Greater Sudbury Police Service officers in two separate cruisers decided to stop a pick-up driver on suspicion that the driver was inebriated. The officers maneuvered their vehicles in front of and behind the truck, and exited their cruisers. Within moments of the stop, the pickup truck operator drove into the rear of the cruiser stopped in front of him, after which he turned the truck to the right, struck a civilian vehicle stopped alongside it, and accelerated away. When the driver entered an intersection against a red light and unsuccessfully attempted to make a left-hand turn, he lost control of the truck and crashed. The man, who ran into a wooded field and then an industrial area, was eventually located with the use of a police dog and taken into custody. Following his arrest, the man was transported to hospital and diagnosed with a minor dog bite to the right buttock and fractures of his left foot. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the injuries.

SIU Concludes Investigation into Hamilton Arrest

In the evening of October 22, 2025, a Hamilton Police Service officer was on patrol in a marked cruiser with another employee when they decided to pull over a pick-up truck for a licence plate issue. The cruiser followed the pick-up truck into an alleyway. The 64-year-old pick-up truck driver exited the vehicle and walked towards a staircase, ignoring the officer’s requests that he stop. The man climbed the stairs to a second-story landing, where he tumbled before the officer caught up to him and told him he was under arrest. A struggle ensued, and the man was eventually arrested. He was transported to hospital where he was diagnosed with a left orbital bone fracture. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Man’s Arrest in North York

On October 25, 2025, the SIU was notified by the Toronto Police Service that an officer had discharged his firearm at a 59-year-old man in the course of the man’s arrest earlier in the day. The man was not struck. The SIU initiated an investigation, naming the officer who discharged his firearm as a subject official. Upon learning that the man had suffered a fractured right elbow, the SIU named a second subject official. The investigation is now concluded. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s arrest.

SIU Concludes Investigation into Fatal Police-Involved Shooting on Wapekeka First Nation

On July 31, 2025, a 23-year-old man passed away, the result of a police shooting on Wapekeka First Nation. The SIU was notified of the incident and initiated an investigation. The investigation is now concluded. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s death.

SIU Concludes Investigation into Man’s Serious Injury Suffered While Lodged in OPP Blind River Cell

A 54-year-old man was arrested on October 19, 2025 and taken to the OPP Blind River Detachment where he was lodged in a cell. Following a video bail hearing the next day, the man was escorted back to the cell area. At the threshold of the door leading to the cells, the man walked in the direction of an employee instead of through the open cell door. An officer approached the man and pushed him away from the employee towards the cell. The man’s back banged up against the cell, pushing the cell door momentarily closed behind him. The officer and the man became involved in a struggle as the employee pulled open the cell door.  The man was pushed into the cell and the cell door closed. When the man complained of pain in his left shoulder, he was transported to a medical facility where he was diagnosed with a serious injury. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s injury.

SIU Concludes Investigation into Man’s Drug Overdose in Toronto Police Custody

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Toronto Police Service officer committed a criminal offence in connection with the drug overdose suffered by a 38-year-old man while in police custody. On October 18, 2025, police responded to a reported assault at George Hislop Park and arrested the man for assault with a weapon.

SIU Concludes Investigation into Man’s Broken Leg in Guelph Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Guelph Police Service officer committed a criminal offence in connection with the fractured left leg suffered by a 35-year-old man. On October 19, 2025, officers went to a residence in the area of Arkell Road and Gordon Street to arrest the man for being in the basement of the house in violation of a release order. He was handcuffed without incident and escorted outside. While out front of the home, the man made a sudden move to his left to break free, and he was forced to the ground by officers.

SIU Concludes Investigation into Cyclist Injured in Collision with Police Cruiser in Sudbury

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Greater Sudbury Police Service officer committed a criminal offence in connection with the fractured collar bone suffered by a 75-year-old man. On October 18, 2025, the man, while riding an electric bike, was struck by the cruiser as he cycled across Lorne Street. The police officer had entered the intersection on a greenlight while en route to a call for service.

SIU Concludes Investigation into Fatal Milton Collision

On March 30, 2024, an 18-year-old male, intoxicated and driving an Infiniti with a 16-year-old female as his front seat passenger, rammed into the rear of a 26-year-old male’s Honda that was stopped for a red light. All three vehicle occupants died in the collision. On July 22, 2024, upon learning from the Halton Regional Police Service that the 18-year-old had twice been stopped for investigation of impaired driving by officers in the couple of hours before the collision, the SIU invoked its mandate. The two officers who conducted those investigations were identified as subject officials. The SIU investigation is now concluded. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that either officer committed a criminal offence in connection with the collision.

SIU Concludes Investigation into Man’s Facial Fractures in Mississauga Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Peel Regional Police officer committed a criminal offence in connection with the fractured nose and orbital bone suffered by a 40-year-old man during his arrest in Mississauga. On October 12, 2025, officers on patrol saw a man pushing an empty wheelchair with an open can of beer on it on Dundas Street East, east of Dixie Road.

On October 9, 2025, officers were dispatched to the Pizza Pizza at 320 Main Street North in Brampton after an employee reported a woman in the store behaving erratically and threatening staff with scissors.

On October 13, 2025, a 43-year-old man lapsed into medical crisis while lodged in a police cell at the Ottawa Police Service station on Elgin Street. The investigation is now concluded. SIU Concludes Investigation into Halton Police Following Man’s Fall from Height

On the night of October 28, 2025, Halton Regional Police Service officers were dispatched to the area of Guelph Line and the Queen Elizabeth Way to investigate a report of a man striking a woman with a skateboard. Upon arriving, officers confronted a man who matched the description and advised him he was under arrest. When one of the officers went to take hold of him, the man fled from the officers on foot. The officers gave pursuit. At one point, the man jumped over a guardrail, not realizing that the ground on the other side was approximately eight metres below. The 25-year-old man was arrested and transported to hospital for treatment of a fractured wrist. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s injury.

On October 2, 2025, the woman was arrested on Dundas Street East for outstanding warrants following a welfare check at a nearby residence.

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Niagara Regional Police Service officer committed a criminal offence in connection with the pelvic fracture suffered by a 42-year-old man in St. Catharines. On October 6, 2025, officers went to a home in the area of Haig Street and Carlton Street following an unknown trouble call, the second 911 call from the man that day. The man was of unsound mind, having taken methamphetamine.

On September 30, 2025, officers went to a residence in Chelmsford to arrest the man. The ankle monitoring device he was wearing gave the arresting officers reasonable grounds to believe he was at the address in violation of a condition of a release order.

On September 29, 2025, police were called to a carpark behind the Guesthouse Shelter, 522 Elizabeth Street, Midland. There were 911 calls about a man armed with a knife chasing a female in the vicinity.

SIU Concludes Investigation into Toronto Police Pursuit

In the early morning of September 21, 2025, a stolen Honda Civic was being operated by a 14-year-old male. Travelling in excess of the speed limit, the vehicle came to the attention of a Toronto Police Service officer. With the officer behind him, the cruiser’s lights and siren activated, the teenager continued to accelerate westbound. Shortly after, the Civic entered into an intersection against a red light and was struck by another vehicle. Arriving at the intersection a couple of seconds later, the officer exited his cruiser and rendered assistance to the parties involved in the collision. The teenager was arrested and taken to hospital where he was diagnosed with a brain bleed.  The occupants of the other vehicle were not seriously injured in the collision. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that the officer committed a criminal offence in connection with the collision.

SIU Concludes Investigation into Woodstock Arrest

In the evening of September 18, 2025, a Woodstock Police Service officer was on patrol when he observed two motorcyclists on Highway 401 riding together without licence plates affixed to their vehicles. Intending to stop them for a Highway Traffic Act infraction, the officer followed them as they exited the highway. The motorcyclists pulled up to a gas pump when in front of them appeared a marked police pick-up truck. One of the motorcyclists accelerated away from the cruiser and left the gas station. The other motorcyclist also attempted to drive away from the scene but he was tackled to the ground by the officer. The man landed awkwardly resulting in him suffering a fractured left ankle. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that the officer committed a criminal offence in connection with the man’s arrest and injury.

No Charges to Issue in Relation to Hamilton Arrest

In the early morning of September 19, 2025, police were called to a vacant building in Hamilton to investigate a reported break and enter in progress. The first officer to arrive exited his cruiser and soon after located a man with a bike beside a wall of the building, and another man in the process of exiting a window feet first. The officer took hold of the man by the chest as he was climbing out the window and told him he was under arrest. He told the man with the bike not to move and directed the other man to lay down. The man refused to get on the ground of his own volition and was forced down by the officer. A struggle ensued, during which time the man’s head struck the wall and began to bleed. The officer called EMS but the man refused treatment. He was cited for trespass and released. Later that day, the man attended hospital and was diagnosed with two left-sided rib fractures. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that the officer committed a criminal offence in connection with the man’s arrest and injuries.

SIU Concludes Investigation into Innisfil Arrest

In the evening of September 17, 2025, South Simcoe Police Service officers were dispatched to a residence in Innisfil to investigate a report of a man assaulting his female partner. Upon arrival, the officers met the woman. The man had left on his motorcycle. As the officers and the woman talked, the man returned. However, he then stopped, turned his motorcycle around, and began to travel away. The officers followed the man and turned on their cruiser’s emergency lights. A few seconds into the pursuit, the officer operating the cruiser pulled alongside the motorcycle. The man was stopped on his motorcycle, its engine still running. The officer exited the cruiser and pushed the man, causing him to tumble with the motorcycle falling on top of him. He was subsequently handcuffed without issue. The man was eventually taken to hospital and diagnosed with a fractured left ankle. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the man’s arrest and injury.

SIU Concludes Investigation into Scarborough Arrest

On the morning of July 2, 2025, a resident of a Scarborough apartment building called 911. She then hung-up the call but not before the 911 call-taker heard yelling in the background. Two Toronto Police Service officers were dispatched to the apartment unit to ensure the caller was okay. En route, they learned that a man was before the courts on a domestic assault charge involving the caller, and that the man was prohibited from being at the address or having contact with the woman. The woman opened the door and allowed the officers entry into the apartment. The man was present. When an officer indicated he would be arrested for breaching his release conditions, a struggle ensued between officers and the man. The man was eventually handcuffed. He was taken to hospital following his arrest and diagnosed with a broken nose. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that an officer committed a criminal offence in connection with the 40-year-old man’s arrest and injury.

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Niagara Regional Police Service officer committed a criminal offence in connection with the fractured right shoulder suffered by a 42-year-old man during his apprehension in St. Catharines. On September 16, 2025, police were called to the Metro on Lakeshore Road in relation to a disorientated man who had fallen in the grocery store.

In the early hours of September 15, 2025, the man was stopped by an officer while for riding a bicycle without a light in the area of Brough Street and University Crescent.

On September 14, 2025, officers went to a residence in the area of Dundas Street East and Parliament Street as the man had reportedly violated a curfew, a term of his release order.

On February 23, 2025, TPS Drug Squad officers went to a house in the area of Eglinton Avenue East and McCowan Road in Scarborough on the strength of a warrant to search for illicit drugs.

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Windsor Police Service officer committed a criminal offence in connection with the fractured nose and orbital bone suffered by a 38-year-old man.

The Deputy Director of the Special Investigations Unit, Stacey O’Brien, has terminated an investigation into the hospitalization of a 44-year-old woman after she ingested cocaine during an interaction with an Ontario Provincial Police officer in Timmins.

SIU Concludes Investigation into Teen Injured During Arrest at Kitchener LCBO

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a Waterloo Regional Police Service officer committed a criminal offence in connection with the fractured right rib suffered by a 16-year-old girl. On September 6, 2025, the off-duty officer saw the girl attempt to steal three bottles of alcohol from an LCBO on Ottawa Street North. The officer tried to arrest her for theft, and she tried to get away. In the entrance of the store, the officer forced her down to the floor and pinned her down while she continued to struggle. He moved her into the store and forced her to the floor again until additional officers arrived.

SIU Concludes Investigation into Motorcycle Collision Involving OPP Cruiser in Welland

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe an Ontario Provincial Police (OPP) officer committed a criminal offence in connection with the serious injuries suffered by a 50-year-old man in a motorcycle collision.  On September 5, 2025, an OPP officer attempted to do a U-turn on Highway 58A while attempting to locate a pedestrian who was reportedly walking in traffic. While he attempted to turn around in a marked Dodge Charger, the operator of a Harley Davidson struck the side of the cruiser. The motorcyclist was taken to hospital with fractures of the ribs, spine and pelvis.

SIU Concludes Investigation into Woman’s Broken Knee in Innisfil Arrest

The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a South Simcoe Police Service (SSPS) officer committed a criminal offence in connection with the fractured left knee suffered by a 40-year-old woman in 2023. The SIU began investigating on September 4, 2025, after the SSPS notified the SIU that they were in possession of information that the woman was seriously injured during her arrest on March 10, 2023.

SIU Investigating Man’s Death in Fall from Toronto Balcony

The province’s Special Investigations Unit (SIU) is investigating the circumstances surrounding the death of a 27-year-old man last night in North York. As Toronto Police Service officers were in the area at the time of the man’s death following a fall from height, the SIU was notified and invoked its mandate.
