SIU Concludes Investigation into Woman’s Death, Serious Injuries of Two Men, in London Collision

Mississauga, ON (6 January, 2026) ---
The Director of the Special Investigations Unit, Joseph Martino, found no reasonable grounds to believe a London Police Service officer committed a criminal offence in connection with a collision that resulted in the death of a 67-year-old woman and the serious injuries of two men in September. 

On September 10, 2025, the officer attempted to stop the driver of a pick-up truck eastbound on Dundas Street after he observed a man riding in the bed of the vehicle. The driver of the pick-up took off and ended up entering an intersection and colliding with an SUV. The driver of the pick-up fled the scene. The officer arrived at the crash site about 14 seconds after the impact, and rendered assistance to the injured.

In the collision, the man, 55, was thrown from the bed of the pick-up truck.  He suffered a collapsed lung and a fractured left-sided rib. The other man, 69, the driver of the SUV, broke his neck, injured his hip and fractured multiple left-sided ribs.  The woman, a passenger in the SUV, succumbed to her injuries on September 17, 2025.

The SIU is an independent government agency that investigates the conduct of officials (municipal, regional and provincial police officers, police officers with the Nishnawbe Aski Police Service, special constables with the Niagara Parks Commission and peace officers with the Legislative Protective Service) that may have resulted in death, serious injury, sexual assault and/or the discharge of a firearm at a person. All investigations are conducted by SIU investigators who are civilians. Under the Special Investigations Unit Act, the Director of the SIU must

  • consider whether the official has committed a criminal offence in connection with the incident under investigation
  • depending on the evidence, cause a criminal charge to be laid against the official where grounds exist for doing so, or close the file without any charges being laid
  • publicly report the results of its investigations
