Notice of Correction to Director’s Report

Case Number: 23-OCD-166   

Mississauga, ON (24 September, 2024) ---
Please be advised that the Director’s Report for 23-OCD-166, originally published on September 17, 2024, has been amended.

More specifically:
  • Under ‘Incident Narrative’, the first sentence of the second paragraph read, “In the evening of May 3, 2024 …” This was replaced with “In the evening of May 3, 2023…”
  • Under ‘Incident Narrative’, the final paragraph read, “On May 7, 2024, the Complainant was pronounced deceased at hospital.”. The year was changed to 2023.
  • Under ‘Analysis and Director’s Decision’, the first two sentences of the first paragraph read, “The Complainant passed away on May 7, 2024. He had lapsed into medical crisis late on May 3, 2024, from which he never recovered.” This has been corrected to “The Complainant passed away on May 7, 2023. He had lapsed into medical crisis late on May 3, 2023, from which he never recovered.”
The SIU regrets the error.

The SIU is an independent government agency that investigates the conduct of officials (police officers as well as special constables with the Niagara Parks Commission and peace officers with the Legislative Protective Service) that may have resulted in death, serious injury, sexual assault and/or the discharge of a firearm at a person. All investigations are conducted by SIU investigators who are civilians. Under the Special Investigations Unit Act, the Director of the SIU must

  • consider whether the official has committed a criminal offence in connection with the incident under investigation
  • depending on the evidence, cause a criminal charge to be laid against the official where grounds exist for doing so, or close the file without any charges being laid
  • publicly report the results of its investigations
