The SIU was notified that Toronto Police Service (TPS) officers were responding to a break and enter call when they saw what they believed to be the suspect car traveling on Carmichael Avenue. The police proceeded to follow the car as it traveled onto Bathurst Street. Near the area of Bathurst and Haddington Avenue, the car struck a Honda Civic.
The 22-year-old driver of the Civic was taken to Sunnybrook Health Sciences Centre with serious injuries and is in critical condition.
The SIU is investigating the extent of police involvement in the events surrounding the collision. Investigators have designated one subject and one witness officer from the TPS and are appealing for anyone who witnessed this incident to contact the Unit at 416-622-2038 or 1-800-787-8529.

All investigations are conducted by SIU investigators who are civilians. Under the Special Investigations Unit Act, the Director of the SIU must
- consider whether the official has committed a criminal offence in connection with the incident under investigation
- depending on the evidence, cause a criminal charge to be laid against the official where grounds exist for doing so, or close the file without any charges being laid
- publicly report the results of its investigations
