Notice of Correction to Director’s Report

Case Number: 24-OOD-047

Mississauga, ON (4 June, 2024) ---
Please be advised that the Director’s Report for 24-OOD-047, originally published on May 24, 2024, has been amended.

More specifically:
  • Under ‘Subject Official’ of ‘The Investigation’, “SO Declined interview and to provide notes, as is the subject official’s legal right”, was replaced with “SO Declined interview, as was the subject official’s legal right”.
  • Under ‘Materials Obtained from Police Service’ of ‘Evidence’, the following bullet point was added: ‘Notes – the SO (PPS)’.
  • Under ‘Incident Narrative’, the second sentence of the first paragraph read, “As was her legal right, the SO chose not to interview with the SIU or authorize the release of her notes.”. This was replaced with: ‘As was her legal right, the SO chose not to interview with the SIU. She did authorize the release of her notes.”

The SIU regrets the error.

All investigations are conducted by SIU investigators who are civilians. Under the Special Investigations Unit Act, the Director of the SIU must

  • consider whether the official has committed a criminal offence in connection with the incident under investigation
  • depending on the evidence, cause a criminal charge to be laid against the official where grounds exist for doing so, or close the file without any charges being laid
  • publicly report the results of its investigations
