Notice of Correction to Director’s Report

Case Number: 24-OCI-166

Mississauga, ON (14 August, 2024) ---
Please be advised that the Director’s Report for 24-OCI-166, originally published on August 12, 2024, has been amended.

More specifically:
  • Under ‘Analysis and Director’s Decision, the fourth sentence of the second paragraph read, “In the instant case, the question is whether there was a want of care on the part of the SO, sufficiently egregious to attract criminal sanction, that caused or contributed to the Complainant’s wounds medical event.” This was replaced with: “In the instant case, the question is whether there was a want of care on the part of the SO, sufficiently egregious to attract criminal sanction, that caused or contributed to the Complainant’s wounds.”

The SIU regrets the error.

All investigations are conducted by SIU investigators who are civilians. Under the Special Investigations Unit Act, the Director of the SIU must

  • consider whether the official has committed a criminal offence in connection with the incident under investigation
  • depending on the evidence, cause a criminal charge to be laid against the official where grounds exist for doing so, or close the file without any charges being laid
  • publicly report the results of its investigations
